- After reviewing the Services and Fees, and when you are ready to move forward you can click the SCHEDULE/REGISTER/PAY button below to start the registration process. (MUST provide full name, email, and phone number)
- START the registration and schedule your appointment. After submission you will receive an EMAIL INVITATION to join your Patient Portal and fill out two questionnaires and personal information.
- MAKE PAYMENT: Once received you will then get confirmation of your APPOINTMENT.

Telehealth Limitations and Disclosure Statement
By participating in this telehealth consultation, you acknowledge and agree that virtual dermatological evaluations possess inherent technical and physical limitations. Telehealth examinations rely entirely on submitted photographic data and real-time video feeds, which lack the diagnostic precision of an in-person clinical exam. Specifically, a remote consultation does not permit physical palpation to assess the depth, texture, or mobility of skin lesions, nor does it allow for professional dermoscopic magnification under controlled, standardized ambient lighting. Consequently, subtle variations in lesion color, border irregularity, or tissue consistency may be obscured or distorted by camera specifications, resolution limits, and user lighting conditions. Telehealth is not a substitute for comprehensive, full-body skin cancer screenings or the evaluation of complex, deep-tissue abnormalities. If a definitive diagnosis cannot be established due to these visual or tactile limitations, you may be directed to seek immediate in-person clinical evaluation.
